This is an exciting opportunity for a qualified CPN (Community Psychiatric Nurse) to join a newly developing Liaison and Diversion (L&D) Team.
The all age Liaison and Diversion service is funded by NHS England, and is currently expanding from its Wakefield base into Elland Road Police Station, Leeds.
It is a multi-agency team working within the Criminal Justice System with the aim of:
- Improving access to healthcare and support services.
- Reducing inequalities in health care provision
- Improving access to other provision
- Reducing re-offending
The Mental Health Practitioner will be required work in a variety of settings across Leeds and Wakefield, including courts, police custody suites, client’s homes and community based settings.
The role of the Mental Health Practitioner involves identifying vulnerable young people and adults coming into contact with the Criminal Justice System. These could be mental health, learning disabilities, substance misuse and other social vulnerabilities.
You will be provide comprehensive assessments, produce and present fast delivery reports for Courts, provide assessments for the National Probation Service and support referrals into appropriate statutory services and third sector organisations.
The service will operate over 7 days from 07:00am until 10:00pm; practitioners will be required to work on a rota basis including evenings and weekends.
